General Responsibility:
Effectively utilizes 3M-Encoder to accurately assign ICD-9CM and CPT-4 codes with a minimum of 95% accuracy.
Abstracts all record with 95% accuracy.
Contacts responsible party for further clarification of documentation as necessary and documents this in the Query log.
Processes all outpatient records accurately to assure proper APC assignment regardless of payor.
Completes coding and abstracting of an average minimum of SDC - minimum of ten (10) per hour, CLI - minimum of thirty (30) per hour, and ER - minimum of thirty (30) per hour.
Assures all accounts are final billed within three (3) days or less, by Friday of each week.
Retrieves records from ancillary departments as needed to assure all records are available for coding.
Monitors and maintains Unbilled Report to confirm that accounts have dropped.
Identifies issues and directs problems with charges and billing discrepancies to the Business Office.
Identifies issues and directs problems with patient statuses and admission/discharge dates to Admitting.
Performs other relevant duties as assigned by the HIM Asst Director or HIM Director.
Fosters teamwork by offering assistance to coworkers, as needed.
Files records in terminal digit order with 99% accuracy.
Files record in correct location with 99% accuracy.
Retrieves all records for physician's to complete.

Requirements:
Proficient in CPT and ICD-9CM coding. At least 2 years coding experience in an acute care facility as an outpatient coder. Special requirements RRA, RHIT, ART, or CCS.
